I hereby represent that all information provided herein are correct and not misleading to the best of my knowledge, and agree that HKBIO may at its discretion access and read the information. I do understand that any wrong information misleading HKBIO may form the basis of rejection of my membership application. I also understand that by applying this membership, I agree to accept the terms and conditions as stated in HKBIO’s Memorandum and Articles of Association and Privacy Policy Statement.

本人現聲明，就本人所理解，此申請表中填寫的全部資料是正確無誤的；並同意香港生物科技協會參考及審閱本人在此申請表中提供的一切資料。本人明白若有錯誤或誤導性的資料，可構成欺騙或不誠實行為，並有機會影響本人之會員申請。本人明白在遞交此申請表即表示本人接受香港生物科技協會之會章及隱私政策。
